--Notes on update

1. Added modal dialogs for both Program and Campaign actions (e.g. Edit, Create, Duplicate, Delete). Note the dialog actions are not yet implemented.
2. Added date to program and campaign menu listings.
3. Fixed problems involving dataset schema choice menus not being removed when re-clicking on the menu buttons (e.g, clicking again on the primary key button above the table heading will make the menu disappear now)
4. Numeric attribute distributions now accurately range between real min and real max.
